Riesgo, mercado y seguridad social en Salud: Revisión de algunos aspectos conceptuales

Abstract

La crisis de los sistemas de seguridad social en salud ha motivado reformas que han buscado expandir la acción del mercado introduciendo elementos competitivos que aseguren mayores niveles de eficiencia. Dichas reformas han tenido que hacer frente a las limitaciones que presenta un mercado de seguros de salud competitivo para agrupar y diversificar el riesgo financiero asociado a una enfermedad. Este trabajo revisa algunos aspectos conceptuales de dichas limitaciones. Se analiza el problema de riesgo de comportamiento por parte de los asegurados y los proveedores de servicios médicos, y se discuten las alternativas contractuales para mitigarlo. Se discuten también las bases de sistemas de competencia administrada que buscan preservar los beneficios de la competencia y evitar el problema de selección de riesgo que limita el acceso a la seguridad de las personas que presentan mayores riesgos en salud y dificulta la existencia de un sistema de seguros de largo plazo.
