La dispersion des coûts entre hôpitaux publics

Abstract

[fre] Si les théoriciens réfléchis sont depuis longtemps convaincus qu'il est impossible de traiter l'hôpital comme une firme, la séduction qu'exercent les modèles d'en­treprise est encore vive et conduit fréquemment à de graves erreurs : l'article explicite donc les raisons pour lesquelles cette référence doit être abandonnée, et tire de cette analyse quelques indications utiles sur la façon d'expliquer la dispersion des coûts hospitaliers. Une réflexion d'ensemble sur les variables explicatives à considérer conduit alors à une analyse statistique. Les résultats, très différents de ceux qui ont été obtenus au Canada dans des conditions com­parables, font apparaître qu'il n'y aurait pas de relation entre les moyens dont disposent les établissements et les tâches qu'ils ont à accomplir, compte tenu des caractéristiques des malades qu'ils reçoivent. Even though reflective theorists have for a long time been convinced of the impossibility to manage the hospital as a firm, the fascination worked by models of companies is still strong and often leads to serions mistakes. The article there-fore specifics the reasons why that reference must be leftf and draws from that analysis some useful information on how to explain the distribution of hospital costs. An overall considération over the explanatory variables to be taken into account then leads to a statistical analysis. The results, much différent from those obtained in Canada in similar conditions, show that there should be no relation between the resources at the disposal of the units and the tasks they have to perform, taking into account the characteristics of the sick they receive.
