Un approccio proattivo e multidisciplinare per la presa in carico territoriale della fragilit? l?esperienza del Distretto di Forl? dell?AUSL della Romagna

Abstract

Nel periodo 2016-2019 presso la Casa della Salute di Forlimpopoli, Distretto di Forl? (AUSL della Romagna), ? stata implementata una metodologia finalizzata alla presa in carico proattiva e multidisciplinare degli adulti pi? fragili, denominata RiskER. Il metodo prevede: analisi di profili individuali di rischio elevato di fragilit?, elaborati da un algoritmo validato, da parte di MMG e infermiere; valutazione dei bisogni delle persone eleggibili per la presa in carico, da parte di MMG e infermiere con il coinvolgimento dell?assistente sociale, sulla base delle informazioni disponibili, e individuazione di potenziali interventi; proposta di intervento alla persona da parte dell?infermiere. I principali interventi sono: arruolamento in percorsi per la cronicit? (diabete tipo II, scompenso cardiaco, BPCO), colloqui per il miglioramento della compliance terapeutica; attivazione del servizio infermieristico domiciliare e/o dell?assistenza sociale.
