Türkiye’de 2002-2011 Yılları Arasında Katastrofik Sağlık Harcamalarının Yoksulluk Yaratma Kapasitesi

Abstract

Hanehalklarının cepten yaptığı sağlık harcamaları, sağlık hizmeti talebinin niteliğine bağlı olarak hanehalkının temel gereksinimlerinden fedakârlık etmesini gerektirebilir. Bu türden harcamalara literatürde katastrofik sağlık harcaması denilmektedir. Katastrofik sağlık harcamalarının en temel özelliği yoksulluk yaratma kapasitesinin yüksek olmasıdır. Çalışmamızda Türkiye’de katastrofik sağlık harcamalarının 2002-2011 yılları arasında yoksulluk yaratma kapasitesini Dünya Sağlık Örgütü ve Dünya Bankası yaklaşımları ile ayrı ayrı hesaplamaya çalıştık. Bu yaklaşımlarla katastrofik sağlık harcamalarının kafa sayısı, katastrofik aşım ve yoğunlaşma endeksleri hesap edilmiştir. Ortaya çıkan bulgulara göre her iki yöntemde de Türkiye’de katastrofik sağlık harcamalarının yeni yoksulluk yaratma kapasitesinin düşük, ama mevcut yoksulluğu derinleştirme kapasitesinin yüksek olduğu bulunmuştur.

    Note: [English Title] Catastrophic Health Expenditures’ Capacity to Create Poverty in Turkey in 2002-2011 [English Abstract] Depending on the structure of demand for health services, the out-of-pocket health expenditure of households may require sacrifice in the household’s basic needs. In the literature, this type of spending is called catastrophic health expenditure. One of the main characteristics of catastrophic health expenditures is the capacity to create poverty. In this study, catastrophic expenditure’s capacity to create poverty in Turkey between 2002-2011 periods is calculated by the approaches of World Bank and World Health Organization. Using these two approaches; the head count indices, catastrophic overshoot indices and concentration indices are calculted. The results of both methods indicate that in Turkey the capacity to create new poverty of catastrophic health expenditure is low however the capacity to increase the intensity of poverty is high.
