Epidemiological and Clinical Profile and Predictive Factors of Mortality from Stroke in Young Adults Admitted to the Medical Intensive Care Unit

Introduction: Stroke in young adults is a growing public health problem, particularly in developing countries, such as India. This study aimed to describe the epidemiological and profile and identify predictive factors for mortality from stroke in young adults admitted to the intensive care unit (ICU).Methods: A retrospective, descriptive, and analytical study was conducted in the Medical ICU and Clinical Toxicology unit of CHU JRA. All patients aged 15–45 years who were hospitalized for stroke confirmed by brain imaging between January 1, 2022, and December 31, 2023, were included. Demographic, clinical, radiological, therapeutic, and outcome data were also analyzed.Results: Of the 429 patients admitted for stroke, 85 (19.8%) were aged 15–45 years. The mean age was 35.9 ± 7.5 years, with male predominance (61.2%). Hypertension (71.8%) was the most common risk factor. Hemorrhagic stroke was predominant (75.3%). The overall mortality rate was 52.9%. Factors independently associated with mortality were impaired consciousness at admission (p = 0.001, OR = 5.31), Glasgow Coma Scale score 20 (p = 0.03, OR = 3.99), time to therapeutic intervention >72 h (p = 0.001, OR = 4.57), presence of cerebral herniation on CT scan (p = 0.004, OR = 4.66), length of stay
