Abstract

Background: Ovarian germ cell tumors (OGCTs) are a rare form of ovarian cancer, especially during pregnancy, and their diagnosis and management present unique challenges. These tumors account for a small percentage of all ovarian cancers, and their diagnosis is often complicated because of non-specific symptoms that overlap with normal pregnancy. Case Presentation: A 26-year-old pregnant woman, G1P0000, presented with an enlarging abdominal mass at 16 weeks of gestation. Ultrasound and CT tomography revealed a solid ovarian mass. The patient was diagnosed with a suspected malignant ovarian germ cell tumor and underwent unilateral salpingo-oophorectomy using frozen sections. Histopathological examination confirmed the tumor to be a mixed germ cell tumor, predominantly the yolk sac and dysgerminoma components. Conclusion: Early diagnosis and management of ovarian germ cell tumors during pregnancy are crucial for ensuring maternal and fetal well-being. Surgical intervention remains the primary treatment option, and fertility preservation of should be considered whenever possible. Post-operatively, the patient recovered well, and further chemotherapy with BEP was planned.
