Amlodipine-induced Peri-implantitis in a Patient with Poor Oral Hygiene: A 10-year Follow-up Case Report

Abstract

Gingival hyperplasia secondary to the administration of calcium channel blockers for the treatment of hypertension in dental implants has not yet been fully investigated. We present a clinical case of peri-implantitis secondary to severe gingival hyperplasia associated with amlodipine administration and its subsequent 10-year follow-up period. Postoperative maintenance therapy is crucial for preventing recurrence of drug-induced gingival hypertrophy. The patient’s commitment to maintaining excellent home oral hygiene, especially with irrigators, and their consistent attendance at every three months check-ups for examination and/or professional ultrasonic cleaning is a “conditio sine qua non” for the prevention of peri-implantitis. Amlodipine-induced peri-implant gingival hyperplasia in patients with poor oral hygiene can progress to amlodipine-induced peri-implantitis, compromising osseointegration of dental implants. However, further studies are needed to validate these findings due to the limitations of a single-case study and confounding factors associated with biomechanics.
