Caracterización de la función renal en pacientes con diagnóstico tardío de VIH/sida. Santiago de Cuba, 2023-2025

Abstract

Introducción: la afectación renal es una de las complicaciones más importantes en las personas con infección por VIH, especialmente en aquellas con diagnóstico tardío, debido a su influencia sobre la evolución clínica y la mortalidad. Objetivo: caracterizar la función renal en pacientes con diagnóstico tardío de VIH/sida según variables epidemiológicas, clínicas, humorales, virológicas e imagenológicas. Métodos: estudio observacional, descriptivo, transversal en el Hospital General Docente Dr. Juan Bruno Zayas Alfonso, Santiago de Cuba, entre junio de 2023 y junio de 2025. La muestra estuvo constituida por 87 pacientes con diagnóstico tardío de VIH/sida. Se estudiaron variables sociodemográficas, factores de riesgo de nefropatía, enfermedades definitorias de sida, indicadores de función renal, alteraciones urinarias, filtrado glomerular, carga viral y hallazgos ecográficos. Los datos se analizaron mediante estadística descriptiva y pruebas de asociación. Resultados: predominaron el sexo masculino (73,6 %) y el grupo de 25 a 44 años (55,2 %). La carga viral superior a 10 000 copias/mL se observó en el 66,7 % de los pacientes. El daño renal estuvo presente en el 34,5 %, siendo la enfermedad renal crónica la forma clínica más frecuente (31,0 %). La proteinuria constituyó la alteración urinaria predominante. Conclusiones: el diagnóstico tardío de VIH/sida se relacionó con una elevada frecuencia de alteraciones renales. La carga viral elevada constituyó un factor estrechamente asociado al deterioro de la función renal, mientras que la enfermedad renal crónica se relacionó con mayor mortalidad hospitalaria.
