Abstract

Introduction: Monolithic zirconia and lithium disilicate are the two materials most used for single-tooth all-ceramic crowns, but their comparative survival and technical complication rates remain uncertain. Methods: PRISMA 2020 systematic review. PubMed, OpenAlex and Crossref were searched from 2019 to 2026. Eligible studies compared monolithic zirconia and lithium disilicate single crowns in adults and reported survival or technical complications. Risk of bias was assessed with RoB 2 and ROBINS-I, and failure and technical-complication proportions were pooled with a random-effects model. Results: Of 550 records, 402 were screened and 11 studies were included (seven randomized trials and four observational studies); one report was not retrieved. Pooled failure was 4.20% (95% CI 1.9-9.0) for zirconia and 6.20% (2.4-14.7) for lithium disilicate, without a significant difference (p = 0.53). Pooled technical complications were 2.80% versus 9.10%. The single five-year comparative study reported survival of 89.2% for zirconia and 84.2% for lithium disilicate (p = 0.067), and one two-year cohort reported 100% versus 82.0%. Conclusions: The available evidence does not show a statistically significant difference in survival between monolithic zirconia and lithium disilicate single crowns. Lithium disilicate showed more ceramic fracture and chipping, but follow-up was short in most studies and the certainty of the evidence is low.
