Abstract

Introduction: Postoperative pain and periapical healing can be influenced by the root canal sealer, and bioceramic (calcium silicate-based) sealers are increasingly recommended over epoxy resin sealers despite contradictory clinical evidence. Methods: A systematic review was conducted following PRISMA 2020. PubMed, OpenAlex and Crossref were searched up to 2023 for clinical studies comparing calcium silicate-based and epoxy resin sealers in permanent teeth. Risk of bias was assessed, and random-effects meta-analyses pooled the standardized mean difference for pain at 24 hours and the risk ratio for periapical healing. Results: From 490 records, 10 studies were included (seven randomized trials and three observational studies). Individual studies mostly found no significant difference in pain; the pooled standardized mean difference at 24 hours was -0.36 (95% CI -0.66 to -0.05; p = 0.021; I² = 2.3%), favoring bioceramic sealers. Periapical healing was similar (pooled risk ratio 1.04; 95% CI 0.93 to 1.16; p = 0.497). Conclusions: Bioceramic sealers are associated with slightly lower 24-hour postoperative pain but similar periapical healing compared with epoxy resin sealers; the certainty of the evidence is low.
