Sglt2抑制劑在合併慢性腎病的心力衰竭患者中的多重獲益研究進展

Abstract

心力衰竭與慢性腎臟病共存是臨床常見表型,這類心腎聯合病變的病理生理機制存在雙向交互作用,會加速病情進展並提升不良臨床結局發生概率,給臨床治療方案制定帶來諸多難題。近年來 鈉-葡萄糖協同轉運蛋白 2 抑制劑(sodium–glucose cotransporter 2 inhibitors,SGLT2 抑制劑)在心血管疾病與腎臟病臨床管理領域獲得學界高度關注,已有相關研究成果逐步表明此類藥物除可通過阻斷腎小管對葡萄糖的重吸收過程實現降糖效果外,還能借助調控腎小球血流動力學特徵、優化機體容量負荷狀況、干預心肌能量代謝途徑等多元作用機制,對心臟與腎臟的生理功能形成潛在保護效應[1]。 合併慢性腎臟病的心力衰竭患者群體相關研究顯示,目前隨機對照試驗與真實世界臨床研究的匯總資料證實 SGLT2 抑制劑的臨床應用與心力衰竭相關不良事件發生風險下降、腎功能衰退速率減慢、機體代謝狀態優化等臨床結局存在明確關聯,此類獲益趨勢在不同類型心力衰竭表型及一定腎功能區間範圍內的患者群體中均有相關研究報導佐證,該類藥物在血糖穩態維持、體重合理管控、血壓水準調節等方面的額外作用效果也為心腎聯合病變患者的整體臨床風險管控工作提供有力支撐[2]。 用藥安全性層面的現有相關研究結論普遍顯示 SGLT2 抑制劑的總體臨床耐受性表現良好,某些特殊臨床場景下仍需重點關注患者機體容量狀態的波動變化、泌尿生殖系統感染的發生可能以及與代謝相關的不良反應風險,需通過科學合理的用藥監測流程與風險管控策略實現不良事件的有效控制。SGLT2 抑制劑在心力衰竭合併慢性腎臟病患者中的臨床應用前景始終受到學界持續關注,該類藥物在不同心力衰竭臨床分型、慢性腎臟病不同病程分期以及存在複雜合併症的患者群體中具體適用範圍與臨床獲益程度仍需要更多後續研究進一步驗證與明確。
