以患者為中心的溝通策略對老年高血壓患者藥物依從性的促進作用

Abstract

老年高血壓是一類患病率長期處於高位且治療週期漫長的慢性病症,其管控成效既需要標準化藥物治療方案的支撐,也在極大程度上取決於患者自身的用藥依從程度。現有研究資料表明,臨床實際診療場景中老年群體常因疾病認知深度不足、對藥物不良反應的過度顧慮、較重的經濟負擔、衰退的記憶能力、有限的家庭幫扶力度以及醫療服務銜接過程中的斷層問題等多重因素,陷入長期用藥的困境,最終對血壓控制效果與相關併發症的防控工作形成顯著不利影響。 以患者為核心的溝通策略強調尊重患者自身價值選擇與個體差異特徵,通過雙向資訊傳遞機制推動患者主動參與治療全流程,該策略近年來已逐漸成為慢性疾病管控領域的核心研究方向,相關文獻研究結論普遍證實這一溝通模式能夠有效提升患者對疾病本質與治療方案的認知水準,緩解其內心焦慮情緒與認知層面的不確定感,同時在特定條件下增強患者落實治療建議的主觀意願與實際執行能力。本文基於現有研究成果的系統梳理,從理論基礎層面切入,全面歸納制約老年高血壓患者用藥依從性的各類關鍵因素,整合分析以患者為核心的溝通策略在臨床實踐中的典型應用形式,具體包含定制化溝通檔案的建立、共情式傾聽與開放式提問的運用、參與式治療決策的推進以及依託隨訪管理與資訊化工具提供的持續性溝通支撐等具體實踐路徑。 現有研究證據的綜合分析結果顯示,基層醫療機構與社區衛生服務場景中以患者為核心的溝通模式在提升用藥認知水準、強化自我管理觀念、優化醫患互動品質等方面具備一定潛在應用價值,同時能夠與家庭及社區層面的支持體系形成協同聯動效應。未來相關研究需在厘清該策略適用場景、幹預力度與實施路徑的基礎上,進一步驗證其在不同人群與服務模式中的實際應用成效,從而為慢性疾病管控工作提供更具實操性的實踐依據。
