Paiement à l’acte et régulation du système de soins : une analyse de longue période (1803-2013)

Abstract

This paper puts forward a retrospective analysis of the health care system ? notably the evolution of pay schemes. The profession was built upon certain age-old principles enshrined in the Medical Charter of 1927. Paradoxically, these principles ? which came about as a result of the war waged by doctors against social insurance in the 1930s ? favoured the development of a socialized medicine. Today socialization is gradually receding and government is trying to phase in new pay schemes which run counter to the Charter?s principles (notably therapeutic freedom). Practitioners would thus be prepared to give up some of these principles to maintain their income levels.
