Asylsuchende und Flüchtlinge in der Notfallstation

Abstract

La présente étude décrit l'état de santé et le traitement des requérants d'asile et des réfugiés du point de vue des médecins du service des urgences de l'hôpital cantonal de St-Gall. Les résultats se basent sur l'analyse de questionnaires et par des fiches de patient documentant les consultations de requérants d'asile et de réfugiés, ainsi que de patients contrôles pendant onze semaines (2×98 consultations). 76% des patients de ces groupes provenaient de l'ex-Yougoslavie. Les diagnostics, codés selon l'ICD, étaient similaires, après correction de l'âge, à ceux des patients contrôles. Par rapport à la clientèle des généralistes, la proportion de blessures était nettement plus importante (37% vs. 8%). Il n'y avait pas de différence entre les patients appartenant aux groupes requérants d'asilelréfugiés, par rapport au groupe de contrôle, quant à leurs classifications comme urgences (58% vs. 65%). De même, il n'y avait pas de différence quant au taux d'hospitalisation (29% vs. 36%) entre les deux groupes. La nuit, les consultations étaient plus souvent considérées comme des urgences que durant la journèe. La durée du séjour des réfugiés en Suisse était négativement associée à un suivi par un médecin de famille et à la classification d'un patient comme urgence. L'analyse a montré que les principales difficultés des médecins provenaient du manque d'expérience dans le traitement des requérants d'asile et des réfugiés, du manque de formations et du manque de routine dans les tâches administratives associées. Par ailleurs, afin d'améliorer l'utilisation des services d'urgences, il serait important d'informer ces patients au sujet du système suisse de la santé et de leur conseiller d'avoir un médecin de famille.
