Evaluaciones económicas en leishmaniasis cutánea: revisión sistemática de literatura 1980—2015

Abstract

La leishmaniasis cutánea presenta elevada morbilidad y pocos tratamientos, los cuales presentan efectos adversos, contraindicaciones y alto costo. El objetivo del texto es describir las evaluaciones económicas realizadas para leishmaniasis cutánea entre 1980 y 2014, a partir de una revisión sistemática en Medline—Pubmed, Medline—Ovid, Science Direct, Scielo, Health Technology Assessment y NHS—Economic Evaluation Database, con un protocolo ex—ante con 12 estrategias de búsqueda, criterios de inclusión, exclusión, evaluación metodológica y reproducibilidad. Se incluyeron 5 estudios, 4 en América, 1 sobre costos de la enfermedad y 4 de costo—efectividad. El costo por ano de vida ajustado por discapacidad (AVAD) fue USD 156 en diagnóstico precoz; USD 1.200, en tratamiento estándar; USD 13.155, en control vectorial y USD 15.215, en control de un brote. El bajo número de evaluaciones económicas y su relevancia para la asignación eficiente de recursos, evidencian la necesidad de incrementar estas investigaciones y priorizar las relacionadas con prevención primaria y secundaria sobre las del tratamiento estándar, dado que las últimas presentan menor costo—efectividad.
