Weiterentwicklung des RSA - Berücksichtigung sozio-ökonomischer Versichertenmerkmale

Abstract

Der Risikostrukturausgleich (RSA) in der gesetzlichen Krankenversicherung (GKV) ist ein "lernendes System" - seit seiner Einführung durch das Gesundheitsstrukturgesetz (GSG) von 1992 ist er mehrfach umfassend reformiert worden. Das GKV-FKG von 2020 sieht vor, dass er regelmäßig auf Performance und Weiterentwicklungsbedarf evaluiert werden soll. Die vorliegende Studie befasst sich mit einer möglichen Weiterentwicklung - der Berücksichtigung sozio-ökonomischer Versichertenmerkmale. Zur empirischen Untersuchung des Themas wird ein großer, an die GKV adjustierter, Datensatz (9,167 Mio. AOK-Versicherte) verwendet. Dieser Studienbericht ist wie folgt aufgebaut: In Kapitel 2 wird eine konzeptionelle Einordnung vorgenommen. In Kapitel 3 wird auf diesbezügliche internationale Erfahrungen eingegangen. Kapitel 4 bildet mit einer empirisch-statistischen Analyse anhand eines großen, an die GKV adjustierten, Datensatzes von AOK-Versicherten den Kern der vorliegenden Untersuchung. Schließlich wird in Kapitel 5 das Ergebnis der Studie diskutiert und es werden Empfehlungen für die Weiterentwicklung des RSA gegeben.
