公的介護保険制度の運用と保険者財政 : 市町村レベルデータによる検証

Abstract

日本の公的介護保険制度は、制度全体の枠組みや価格設定、サービス内容といった競争条件は国によって統一的に決められている反面、保険者である市町村（あるいはその広域連合）の財政状況によって、本来統一的に運用されるはずの要介護認定や介護保険を通じたサービスの利用に格差がみられるのではないかと指摘されている。本稿では市町村レベルのデータを用いて、保険者の財政状況が要介護認定やサービスの利用に及ぼす影響について、定量的な検証を試みた。実証分析の結果、財政状況が悪化している保険者では、介護保険を通じた介護サービスの利用の前提となる要介護認定率や利用者数の増加が、有意に低くなってきていることが明らかになった。
