介護保険導入による介護サービス利用可能性の拡大が社会的入院に与えた影響

Abstract

2000年4月に施行された介護保険制度導入の1つの目的は社会的入院を是正し医療費を効率化させることにあった。この論文では、介護保険導入による介護サービスの利用可能性の拡大に焦点をあて、それが社会的入院患者の退院選択に与えた影響を退院確率の変化から分析した。用いたデータは、富山県の国民健康保険老人医療の入院レセプトデータから、介護保険導入前後の約5年間において、疾病コードの情報が得られる3,043人のパネルデータである。入院患者の入院先医療機関が提供する3つの介護サービス― ― ― 介護療養型医療施設，デイケア，そして、介護老人保健施設― ― ― の利用可能性拡大が入院患者の退院行動にどのような影響を与えたかをLog-logisticハザードモデルを使用して分析した。分析の結果、介護保険導入による介護療養型医療施設の病床増加は、比較的低密度の医療行為しか行われていない入院患者や、長期入院の傾向にある患者の退院確率を高めたことがわかった。特に、2002年度以降180日超入院の特定療養費化に伴い付加された、長期入院患者を介護療養型医療施設へ移行させる誘因となる条件により、介護療養型医療施設の病床増加は長期入院患者の退院確率を大幅に高めた。この結果からのインプリケーションは、第一に、介護保険導入による介護療養型医療施設の病床増加は社会的入院患者の可能性が高い患者の退院を促進した。第二に、長期入院患者が介護療養型医療施設などの介護サービスへ移行することに関して診療報酬上のメリットを提示すると、社会的入院患者の可能性が高い患者の介護サービスへの移行は大幅に進む、ということである。
