Abstract

Cette recherche-intervention porte sur la gestion du médicament au sein du site d'un CHU (Centre hospitalier universitaire) et l'analyse de ses dysfonctionnements organisationnels, ainsi que la mise en place et l'évaluation de projets de changements co-construits entre pharmacie et services cliniques, pour améliorer la performance du circuit du médicament au bénéfice final de la sécurité des patients. Une phase d'écoute préalable des acteurs, suivie d'un dispositif de co-construction de solutions d'améliorations a permis de mobiliser pharmacie et services cliniques. Les actions mises en œuvre et les indicateurs ont amélioré partiellement la coopération. Au-delà, c'est la place de la pharmacie et du pharmacien qui sont ré-interrogés.
