Reformas al sistema de salud en Chile: desafíos pendientes

Abstract

Resumen Las reformas realizadas a inicios de los años ochenta cambiaron la estructura y funcionamiento del sector salud en Chile. Entre los principales cambios se encuentran la creación del Fondo Nacional de Salud; del Sistema Nacional de Servicios de Salud; de las Instituciones de Salud Previsional ISAPRE; y el proceso de municipalización de la atención primaria. En este trabajo se presenta una descripción del modelo de financiamiento del sector salud en Chile. Se argumenta la necesidad de redefinir la mezcla pública-privada, para permitir mayores grados de solidaridad a nivel del financiamiento, reducir el problema de la selección de riesgos y permitir una mejor articulación entre el sector privado y el sector público. Para ello es necesario avanzar hacia seguros de salud universales e integrales. En este contexto, se debe avanzar en la introducción de mecanismos de solidaridad mediante la creación de un fondo solidario, encargado de la captación de los recursos financieros. Este fondo no es incompatible con la existencia de múltiples aseguradoras, privadas o públicas, que administren y gestionen los seguros de salud.
