Structured methodology
This structured methodology helps to provide a structure for incident-accident analysis within the hospitals and to standardise data collected on undesirable events, their causes, contextual factors, and the management of the situation. It can help to evaluate and collect data on incidents and accidents occurring in hospitals and to create database at local and national level.
The structured methodology includes 10 chapters:
1. REPORT FORM (AH-223 incident/accident report);
2. A CONCEPTUAL FRAMEWORK for thorough analysis;
3. A STEP BY STEP METHOD to conduct a thorough investigation;
4. A list of TRIGGER CRITERIONS for thorough analysis;
5. ADVICE FOR THE COMPLETION OF THE THOROUGH ANALYSIS;
6. A description of the REPORT OF THOROUGH ANALYSIS;
7. A DATA ENTRY FORM OF RESULTS OF THOROUGH ANALYSIS;
8. USER GUIDE OF DATA ENTRY FORM;
9. APPENDICES;
10. TOOLBOX (tools, which aid in the completion of the analysis and checklist sheets).
Checklist sheets
The sheets have to simplify the trigger and the completion of the analysis. The analyst can always consult the structured methodology if needed.

Nous avons développé un outil pour aider à la conduite des analyses approfondies d’incidents-accidents, appelé DANAISS (Déclaration et ANalyse approfondie des Accidents et Incidents survenus dans le Système de Soins). Cet outil comprend un document de référence et un ensemble de 6 fiches aide-mémoire.
Le document de référence
L’objectif du document de référence est d’une part, d’aider à structurer au sein des établissements la conduite des analyses approfondies des accidents et incidents liés à la prestation des soins et d’autre part, de standardiser l’identification des événements indésirables et l’analyse des causes, des facteurs de contexte et de la gestion des situations à risque pour permettre un retour d’expérience et une remontée de certaines informations au niveau local (registre local) puis à terme pour l’ensemble des établissements de la province (registre provincial). Le document de référence comprend 10 chapitres :
1) Le FORMULAIRE DE DÉCLARATION d’incident-accident (utilisé par les établissements);
2) Le CADRE CONCEPTUEL proposé pour la conduite d’une analyse approfondie;
3) Une DÉMARCHE structurée en 7 étapes pour organiser les analyses approfondies;
4) Une liste de CRITÈRES DÉCLENCHEURS de l’analyse approfondie;
5) Une AIDE À LA CONDUITE DES ANALYSES APPROFONDIES (avec principes méthodologiques, conseils et une liste de questions);
6) Une description du DOSSIER D’ANALYSE APPROFONDIE;
7) Un FORMULAIRE DE SAISIE DES RÉSULTATS DE L’ANALYSE APPROFONDIE (papier);
8) Le GUIDE D’UTILISATION DU FORMULAIRE DE SAISIE;
9) Des ANNEXES (le texte du projet de loi 113 et un exemple d’analyse détaillée);
10) Une BOITE À OUTILS (fiches descriptives d’outils pouvant être utilisés, par exemple le diagramme d’Ishikawa et un jeu de fiches aide-mémoire).
Le jeu de fiches aide-mémoire
Les fiches doivent permettre de simplifier le déclenchement et la conduite de l’analyse. L’analyste peut toujours consulter le document de référence si besoin.
