Abstract

Zusammenfassung Die Medizinische Dokumentation dient seit jeher als Erinnerungs-, Kommunikations-, Entscheidungs- und Organisationshilfe. Allgemeines Ziel ist es, die richtige Information, das richtige Wissen und die richtigen Kenndaten zum richtigen Zeitpunkt am richtigen Ort den richtigen Personen in der richtigen Form zur Verfügung zu stellen. Voraussetzung für eine gute Dokumentation ist die Dokumentationsplanung. Zur Umsetzung der Medizinischen Dokumentation, dem Grundgerüst von Health Data Management, stehen verschiedene Methoden, Techniken und Werkzeuge zur Verfügung. Große Bedeutung haben dabei Klassifikationen und Nomenklaturen. Standardisierte Dokumentationen wie in der MII oder dem NUM bieten neue Mehrwerte bezüglich der einrichtungsübergreifenden Datennutzung. Eine durchgängige strukturierte Dokumentation ist aber aufgrund der hohen Aufwände nicht umsetzbar. Künftige Arbeitsgebiete sind daher u. a. die Suche nach vereinfachten Methoden der Daten- und Texterfassung und die Auswertung von unstrukturierten Informationen.
