Abstract

El objetivo de este informe es contribuir al debate sobre la financiación territorial sanitaria en sus tres grandes ejes: estimaciones de ingresos públicos y de gasto sanitario, de la necesidad relativa y del impacto sobre la equidad del sistema. Tras siete años de funcionamiento del modelo, se pone en evidencia la sostenibilidad del sistema de financiación si no se producen ajustes verticales que recojan la evolución de las necesidades de los diferentes niveles de Gobierno, especialmente en un momento de desaceleración recaudatoria. El Consejo de Política Fiscal y Financiera (CPFF) alcanzó finalmente un acuerdo para la reforma del sistema de financiación de las comunidades autónomas en julio de 2009. El nuevo sistema se orienta hacia el criterio de necesidad, perfeccionándolo mediante una mejor adecuación de la financiación a las necesidades reales de gasto, revisando los criterios de ajuste de la población. Aumenta, además, la autonomía y la corresponsabilidad fiscal, considera explícitamente la nivelación de los servicios públicos fundamentales y crea fondos específicos para la convergencia real entre las comunidades autónomas.
