Welche Umverteilungseffekte deckt das Solidaritätsprinzip in der GKV ab?

Abstract

Der demografische Wandel führt den in der umlagefinanzierten Krankenversicherung angelegten Risikoausgleich zwischen Jung und Alt ad absurdum. Zwar bleibt die Solidarität im Querschnitt der Versicherten auch bei steigenden durchschnittlichen Leistungsausgaben in jeder einzelnen Periode gewahrt. Über einen längeren Zeitraum werden aber die jeweils nachrückenden Kohorten stärker belastet. Ähnliches gilt für den Sozialausgleich zwischen leistungsfähigen und weniger leistungsfähigen Versicherten. Um auch der intergenerativen Dimension des Solidaritätsprinzips gerecht zu werden, sollten deshalb Umlageverfahren und Anwartschaftsdeckung miteinander kombiniert werden.
