Les hôpitaux sous l'Ancien Régime : des entreprises difficiles à gérer?

Abstract

[fre] Résumé Si les problèmes d'administration et de gestion hospitalières ont été abordés dans le cadre de monographies, ils ont rarement donné lieu à des tentatives de synthèse et encore moins remis en cause l'idée d'une gestion routinière et archaïque. À partir de travaux réalisés sur les budgets d'une centaine d'hôpitaux français et de recherches plus approfondies sur l'administration des hôpitaux picards, il nous a semblé utile d'essayer de peser, avec précaution et sur la longue durée, les principaux chapitres de recettes et de dépenses. Comme l'essentiel des ressources (2/3 environ) étaient assurées par les revenus patrimoniaux, il était inévitable, au-delà de l'extrême variété des statuts et des situations, que l'ensemble des gestionnaires, religieuses économes comprises, aient cherché à conserver et à rationaliser l'exploitation de leur patrimoine, de plus en plus convoité à la fin de l'Ancien Régime. Gestion prudente, scrupuleuse, pragmatique qui n'a pas toujours su ou pu investir mais qui a souvent évité le pire et permis aux hôpitaux de remplir leur mission charitable avec les moyens de l'époque. [eng] Abstract If the problems of hospital direction and management have been studied in monographs, they have seldom induced at synthesis and still less questioned the idea of a routine and archaic management. From studies about the budgets of a hundred or so french hospitals and through research on the hospitals of Picardy, it appears useful to estimate, with caution on the long term, the main chapters of incomes and expenses. While the greatest part of ressources (about 2/3) are provided by the patrimonial incomes, it was unvoidable, beyond the extreme variety of status and cases, that all the managers, treasurer-nuns included, tried to keep and to rationalize their patrimony which was more and more coveted at the end of the Ancien Regime. This cautious, rigourous and pragmatic management was not always able to invest but was often able to avoid often the worst and to allow the hospitals to perform their charitable mission with a few means.
