Organizzazione della Struttura Complessa di Medicina interna per intensit? di cure: l?Area critica di Medicina interna

Abstract

Negli ospedali con organizzazione tradizionale, cioe basata su reparti specialistici non organizzati per intensita di cure, i pazienti "mediamente critici" a volte sono ricoverati in rianimazione, ove ricevono un eccesso di prestazioni con spreco di risorse, a volte vanno nei comuni reparti, ove pero creano effetti distorsivi, in quanto attraggono la gran parte delle risorse, sottraendo assistenza agli altri pazienti, pur ricevendo un?intensita di cure inferiore alle loro necessita. Per evitare queste distorsioni e opportuno creare aree assistenziali di tipo semintensivo, quale l?Area critica di Medicina interna. Gli autori, dopo analisi della letteratura sull?organizzazione degli ospedali per intensita di cura, sottolineano i vantaggi dell?applicazione di tali principi non all?intero ospedale, ma all?interno dei Dipartimenti o delle Unita Operative di Medicina interna. Partendo da alcune esperienze gia realizzate in maniera spontanea in singole strutture con l?attivazione di alcuni posti letto monitorati di terapia semintensiva, propongono un progetto applicativo strutturato per la creazione dell?Area critica di Medicina interna, redatto con l?utilizzo di alcuni strumenti del project management (WBS, OBS, diagramma di Gantt, matrice delle responsabilita, analisi SWOT).
