Zwischen Kostenprivatisierung und PatientInnenautonomie: Eigenverantwortung in der Gesundheitspolitik

Abstract

Dieses Papier verfolgt zwei Ziele: Zum einen geht es darum, anknüpfend an häufig gebrauchte theoretische und argumentative Stränge zur Eigenverantwortung in der Gesundheitspolitik (z.B. Moral-Hazard-Theorie) Möglichkeiten, aber auch Grenzen der Eigenverantwortung in der Gesundheitspolitik auf abstrakter Ebene auszuloten. Inwiefern kann die Einführung eigenverantwortlicher Elemente in das Gesundheitswesen die Effizienz und Effektivität sozialpolitischer Leistungserbringung erhöhen und welche prinzipiellen Hindernisse (beispielsweise durch Informationsasymmetrien der PatientInnen gegenüber dem Arzt; anbieterinduzierte Nachfrage) stehen der Verlagerung von Verantwortung auf die BürgerInnen entgegen? Im zweiten Teil des Papiers analysieren wir vor diesem Hintergrund die Reformentwicklungen im deutschen Gesundheitssystem seit Anfang der 1990er Jahre. Der Beitrag schließt mit einigen Überlegungen zur Umsetzung eines Konzepts der Eigenverantwortung, das Autonomie und Selbstbestimmung in der Lebensgestaltung der BürgerInnen als Ziele in den Vordergrund stellt.
