Avaliação de Eficiência em Sistemas de Saúde: Brasil, América Latina, Caribe e OCDE

Abstract

Este trabalho dá sequência à série de estudos desenvolvidos no Ipea, visando comparar a eficiência do sistema de saúde brasileiro com os sistemas de saúde de outros países. Realizamos avaliações de eficiência que procuram inferir em que medida o investimento, em termos monetários, no sistema de saúde do Brasil, comparado com os sistemas de saúde dos países da Organização para a Cooperação e Desenvolvimento Econômico (OCDE), da América Latina e Caribe seria eficiente em maximizar indicadores relacionados ao desempenho desses sistemas. Em resumo, neste trabalho estamos unificando as duas bases de dados utilizadas nos primeiros textos da série. As metodologias básicas utilizadas foram a Análise Envoltória de Dados - Data Envelopment Analysis (DEA) - e as fronteiras estocásticas - stochastic frontiers (SFs). A posição relativa do nosso país (e de outros países) varia bastante em função do modelo de análise, das variáveis consideradas em cada modelo, e da amostra de países utilizada. Portanto, devem ser evitadas afirmativas enfáticas, categóricas ou pretensamente definitivas sobre o desempenho relativo de nosso sistema de saúde. Trata-se de questão ainda em aberto na literatura. This work follows the series of studies conducted in Ipea, in order to compare the efficiency of the Brazilian health system to health systems of other countries. We evaluate the efficiency of expenditure in health care provision by comparing various output measures (life expectancy; infant survival rate; years of life recovered from diseases; population; area) from the health system of Brazil, Latina America, Caribbean Countries and Organisation for Economic Co-operation and Development (OECD) countries with an expenditure measure (health expenditure per capita). In short, we have merged the two databases of our antecedent works. When estimating the efficiency frontier and country rankings we combine data envelopment analysis (DEA) with stochastic frontiers (SFs). We show that the results we have for Brazil and other i. We believe that much caution is required when performing efficiency analysis across countries. This question is still open in the literature.
