Påvirker forsknings- og undervisningsinnsatsen kostnadseffektiviteten ved norske sykehus? Analyse av paneldata 1999-2001

    Abstract

    Tema for denne rapporten er fordeling av behandling, forskning og undervisningsoppgaver (FoU aktiviteter) på sykehusene. Data for forsknings- og undervisningsproduksjon fra 1999 til 2001 er undersøkt, samtidig med tall for pasientbehandling og innsatsstyrt finansiering (ISF). Tradisjonelt ivaretar sykehusene tre oppgaver. Først og fremst behandler de pasienter, de lærer opp og underviser helsepersonell, og de driver en utstrakt forskningsvirksomhet. Disse oppgavene er sammenfiltrede og avhengige av hverandre. Sykehuset kan vanskelig drive forskning og undersvisning uten pasienter siden disse aktivitetene er avhengig av observasjon av sykdom, behandlingsmetoder og praksis i pasientbehandling og vice versa. Ikke uventet viser undersøkelsen at de store sykehusene har størst forskningsaktivitet, Rikshospitalet er øverst på listen, deretter følger Ullevål, Haukeland og Radiumhospitalet. Også når det gjelder undervisning har de største sykehusene stor aktivitet, men også mindre sykehus bidrar. I snitt har et somatisk sykehus 1537 praksisuker for høyskolekandidater. For undervisning av turnuskandidater og assistentleger er tendensene de samme. I snitt har et somatisk sykehus 13,5 årsverk med turnuskandidater (leger, jordmødre og fysioterapeuter) og 44,1 årsverk for assistentleger. Pasientkostnadene varierer fra sykehus til sykehus, og fra år til år. Undersøkelsen viser at forskning og delvis undervisning reduserer sykhusenes kostnadseffektivitet for pasientbehandling. Tallene gir derimot kun svak støtte til en hypotese om at ISF har medført vridning av sykehusenes innsats over til pasientbehandling på bekostning av forsknings- og undervisningsvirksomheten.
