Abstract

La littérature économique a longtemps stigmatisé les activités de service, en n'hésitant pas à recourir, de manière métaphorique, à une terminologie médicale. Ainsi, non seulement ces activités souffriraient d'un certain nombre de pathologies plus ou moins graves (« cost-desease », « syndrome de Solow », etc.), mais elles mettraient en danger, en les affaiblissant, les organes vitaux du système économique, que sont les industries manufacturières, seules véritables créatrices de richesse, et seules authentiques activités innovatrices. Le service hospitalier résiste, d'une certaine manière, à ce diagnostic. En particulier, l'association des termes innovation et hôpital ne semble pas incongrue. Au sein du secteur tertiaire, les services hospitaliers (et les services médicaux en général) occupent donc une position particulière, dans la mesure où personne ne conteste ni leur utilité sociale, ni leur importance symbolique (le rapport à la vie et à la mort) et leur prestige, ni leur aptitude à la recherche et à l'innovation. Cependant, notre hypothèse est que cet effort d'innovation est sous-estimé voire méconnu dans certains cas. Il l'est d'autant plus que l'on s'éloigne des activités traditionnelles des professions savantes, qui produisent des innovations médicales (de process ou de produit) fortement « science-push ». L'objet de ce travail est de tenter de fournir une grille générale d'analyse de l'innovation à l'hôpital, qui devrait permettre de comprendre l'innovation dans l'hôpital dans toute sa diversité. Au total, ce travail est organisé en trois chapitres. La chapitre un est consacré à un bilan de la littérature sur l'innovation à l'hôpital. On peut dire que l'essentiel de cette littérature est dominé par ce que nous appelons les approches technologistes, c'est-à-dire centrées sur l'introduction de systèmes techniques : initialement les systèmes techniques médicaux, puis de plus en plus également les systèmes techniques de gestion (informatique en particulier). Dans le chapitre deux, nous proposons une grille d'analyse de la prestation hospitalière envisagée comme prestation de service complexe, dont la fonction médicale à proprement parler est insérée dans un système complexe d'autres services élémentaires. La qualité et la dynamique (au sens de l'aptitude à l'innovation) de ces services exercent des effets considérables sur l'efficience de la prestation médicale au sens strict. Nous opérons, ainsi, dans ce chapitre, une transformation de la grille d'analyse du produit hospitalier en une grille d'analyse de l'innovation et des logiques de l'innovation à l'hôpital. Le chapitre trois a un statut particulier. Il peut être considéré comme un chapitre annexe sur le plan formel. Cela ne signifie pas (bien au contraire) qu'il soit moins important à nos yeux. Ce chapitre est consacré à la présentation (plus ou moins détaillée) d'un certain nombre de cas d'innovations. Il s'appuie à la fois sur la littérature spécialisée et sur une enquête par entretiens approfondis au sein de plusieurs établissements hospitaliers. Ce chapitre d'études de cas peut ainsi être lu indépendamment de la grille analytique qu'il cherche à valider.
