ブレア政権の医療改革によるnhsの財政構造の変化ー健全性・公平性の観点による考察ー

Abstract

本稿の目的は,英国ブレア労働党政権の医療改革が財政にどのような影響を与えたか, 健全性・公平性の観点から検証することにある。先進諸外国は福祉国家として財政を維持 するのが難しくなってきており,医療政策は重要課題であることと,小泉政権は保守党政 権を参考にした部分が多く,ブレア政権をみることで,これからの日本がみえるのではな いかと考えるからである。英国は 2003 年度に医療費を拡大したが,2004 年度から赤字に 転落し,2006 年度にV字回復した。原因は人件費の増大と予算配分の変更である。政権公 約のうち職員増については目標以上に達成しており,これも財政赤字の原因であった。し かし一方で、医療費拡大で一人当たりの医療水準が上がり,低所得者が受けられる医療範 囲が増え,公平性を高めていた。保守党政権は現金給付に力を入れていたが,ブレア政権 は医療を含めた現物給付に力を入れていたことも分かった。
