    Abstract

    [eng] Cost-efficiency methods for resource allocation décisions in health care lay on fondations about the functioning of health care system. These foundations can be contestable and this put on question the use of results of these methods. It is then necessary to consider non-standard methods of appraisement in health care in which the results of cost- efficiency methods could be used as informations among others. [fre] Le développement et le succès croissant des méthodes standard de l'évaluation coût-avantage ne doivent pas dispenser d'une réflexion sur leurs fondements. La démarche optimisatrice, les caractéristiques du domaine étudié (externalités, rigidités institutionnelles, incertitude, asymétrie d'information...) impliquent que ces méthodes relèvent d'une conception standard élargie du système qu'elles sont censées évaluer. Les conclusions obtenues par ces méthodes sont relativement opérationnelles, mais restent soumises à des limites méthodologiques et éthiques. S'ajoute à ces limites le caractère contestable des fondements de l'approche standard élargie dans le domaine de la santé. Il convient alors d'envisager des méthodes non standard de l'évaluation qui puissent utiliser les résultats des méthodes coût- avantage comme des informations parmi d'autres intégrées dans une procédure de négociation par apprentissage.
